A $50 deposit is required for the initial consultation and mock up of the tattoo design.

Let's Get Started! 

1. Firstly, please fill out this Initial Questionnaire and Consent form so that I can gain a better understanding of the design that you are looking to have tattooed. 

2. Secondly, please fill out the Liability and Consent form as well as the Cancellation Policy form. These forms are automatically emailed to me once they are filed out, so there us no need to email them to me directly. 

3. I will be in touch with you within 5-7 business days after I have had time to look at your Questionnaire form and we can schedule an appointment together either in person or remotely.

Must be 18 years old. 

No alcohol before the appointment. Your body will not retain the ink as well because you are more prone to heavy bleeding.
